Terms of Service

These terms apply to your use of futureofhealthnp.com and to any service you purchase from Future of Health, doing business as The Future of Health with Rebecca NP ("we", "us"). By using the site or buying a service, you agree to them. If you do not agree, please do not use the site.

1. What we offer

We provide functional and root-cause health care by telehealth, delivered by Rebecca Crawforth, MSN, APRN, FNP-C, a board-certified nurse practitioner. Our services currently include:

• the Root Cause Call, a complimentary 20-minute conversation
• the Comprehensive Health Assessment, which includes an advanced biomarker panel, a review of your history, a 90-minute one-to-one consultation, and a written summary
• The Vitality Code, an ongoing programme entered by separate agreement
• books and written materials, sold through third-party retailers

This is a private, cash-pay practice. We do not bill insurance and we are not in network with any insurer.

2. Not medical advice, and no provider-patient relationship

Do not disregard, delay, or discontinue medical advice from your own provider because of anything you read here. If you are experiencing a medical emergency, call 911 or go to your nearest emergency department. This practice does not provide emergency or urgent care and does not monitor messages outside business hours.

3. Where we can provide care

Clinical services are provided only to clients physically located in states where Rebecca Crawforth holds an active nursing license at the time of the visit. Licensure is expanding, and the states covered may change.

You are responsible for telling us where you are located, and for telling us if that changes. If you are outside a licensed state, we cannot provide clinical care, and any purchase made in those circumstances will be refunded in full.

4. Telehealth

Care is delivered remotely, over secure video. Telehealth has real limits: no physical examination is performed, and technology can fail. By booking, you accept those limits and agree that a referral for in-person evaluation may sometimes be the right recommendation.

If a technical failure interrupts a visit, we will resume it or reschedule at no additional cost.

5. Booking, payment, and refunds

Payment

The Comprehensive Health Assessment is $799, payable in advance through Stripe. Your purchase is confirmed when payment is received. Prices may change, but the price shown at the time of your purchase is the price you pay.

Refunds

If we cannot provide care for a reason on our side, including a licensure restriction or a clinical decision that we are not the right fit, you receive a full refund regardless of stage.

Rescheduling and missed appointments

Please give at least 24 hours' notice to reschedule a consultation. Appointments cancelled with less notice, or missed without notice, may be treated as delivered. Life happens, and we would rather hear from you than not, so tell us as early as you can.

HSA and FSA

Many clients pay using an HSA or FSA. Eligibility depends on your individual plan and is decided by your plan administrator, not by us. We can provide a receipt. We cannot guarantee that your plan will accept it.

Laboratory costs

The Assessment fee covers the panel described at the time of purchase. Any additional or specialty testing you choose to add is billed separately by the laboratory or testing company, and is your responsibility.

6. Your responsibilities

• Give complete and accurate information about your health, medications, and history. The quality of what you receive depends on it.
• Keep your own primary care provider, and keep them informed.
• Follow up on anything we recommend you take to a conventional provider or specialist.
• Attend appointments, or tell us in advance if you cannot.
• Use the site and services lawfully, and do not attempt to disrupt or gain unauthorised access to them.

7. Results and testimonials

Health outcomes depend on factors we do not control, including your history, your circumstances, and what you do with the information you receive. Individual results vary. Any client experience, testimonial, review, or example shared on this website is one person's experience. It is not typical, not predictive, and not a guarantee that you will experience anything similar.

We do not promise a diagnosis, a cure, a resolution of symptoms, or any specific outcome.

8. Intellectual property

The content of this website, together with our written materials, protocols, frameworks, assessment summaries, and the names Future of Health, The Vitality Code, Comprehensive Health Assessment, Root Cause Call, and Eat to Optimize Method, belong to us.

You may read this website and use materials provided to you for your own personal health. You may not copy, republish, resell, or distribute them, use them to build a competing service, or use our name or branding without written permission. The Ultimate Health & Wellness Guide is a published work sold through third-party retailers and is subject to copyright.

9. Third-party services and links

We use third parties to run the business, including Stripe for payment, Calendly for scheduling, Squarespace for the website, and independent laboratories for testing. This site also links to other websites, including Amazon. We are not responsible for the content, terms, availability, or privacy practices of any of them.

10. Disclaimer of warranties

The website and its content are provided as they are, without warranties of any kind, express or implied, to the fullest extent the law allows. We do not warrant that the site will be uninterrupted, error free, or free of harmful components, or that the information on it is complete or current.

This section limits warranties about the website and its content. It does not limit the professional standard of care owed to you as a client, which is not something that can be disclaimed and which we would not attempt to disclaim.

11. Limitation of liability

To the fullest extent permitted by law, we are not liable for indirect, incidental, special, consequential, or punitive damages, or for lost profits or lost data, arising out of your use of this website or its content. Our total liability arising out of or relating to the services is limited to the amount you paid us for the service giving rise to the claim.

Nothing in these terms limits liability that cannot lawfully be limited, including liability for gross negligence, wilful misconduct, or professional negligence in the provision of clinical care.

12. Indemnity

You agree to indemnify us against claims, losses, and reasonable legal costs arising from your misuse of the website, your breach of these terms, or your violation of the rights of another person.

13. Governing law and disputes

These terms are governed by the laws of the State of Utah, without regard to its conflict of law rules. Any dispute arising out of or relating to these terms or the services will be brought in the state or federal courts located in Utah, and you consent to that jurisdiction.

Before starting any formal proceeding, please contact us at info@futureofhealthnp.com and give us a genuine opportunity to resolve the matter. Most things can be sorted out with a conversation.

14. Changes to these terms

We may update these terms as the business changes or as the law does. The date at the top shows when they were last revised. Changes apply to use of the site and to purchases made after they are posted. The terms in effect at the time of your purchase govern that purchase.

15. General

If any provision of these terms is found unenforceable, the rest remain in force. Our not enforcing a provision on one occasion does not waive it. These terms, together with our Privacy Policy and any separate agreement you sign for a programme, are the entire agreement between us regarding the website and the services described here.
